Service Description:
Qualtrics is a full-featured web-based application for creating and conducting online surveys and forms. Qualtrics is available for use by the Tufts community under a university-wide license.
Service Notes:
- All use of Qualtrics must be in support of the University's administrative, teaching, and/or research work.
- Researchers: Surveys that involve human subjects may require review by Tufts' Institutional Review Board (IRB) for Medford surveys or Tufts' Scientific Review Committee (SRC) for health sciences surveys. Please review these policies carefully and obtain permission, if necessary, before conducting a survey.

- Schools and Divisions: Arts & Sciences, Central Administration, Engineering, Fletcher, Friedman School of Nutrition, Science and Policy, Sackler School of Graduate Biomedical Sciences, Tufts Cummings School of Veterinary Medicine, Tufts School of Dental Medicine, Tufts School of Medicine
- Tufts' license does not extend to affiliate organizations unless use of the license is in support of research that originated within an academic unit at Tufts University.
Access instructions:
Qualtrics is an online application that can be accessed through any internet browser using the Tufts Qualtrics link. Log in with your Tufts Username and Tufts Password.
